ZERO ISCHEMIA ROBOTASSISTED HILIAR RENAL TUMOR

Laparoscopic partial nephrectomy can be particularly challenging for more complex tumors, such as, large, endophytic, or hiliar masses. In theses cases open approach is still frequently used. Robotassisted partial nephrectomy (RPN) can be regarded as an evolution of LPN, as the application of the Da Vinci system allows surgeon to overcome technical challenges such as tumor escision and renorraphy. We describe in this video a robotic approach to a right hiliar renal tumor without vascular clamping. Procedure was performed in a 64 years old man with an incidental finding of a solid tumor of 3,5 cmts with necrosis areas located in right renal hilium (PADUA score 10 and RENAL score 9).
Robotic partial nephrectomy provides feasible comparable surgical outcomes in moderate and complex renal tumors. Hight-definition, three dimensional optics and wristed instruments allow advantages for approaching more difficult cases that probably were suitable for radical nephrectomy or open partial nephrectomy approach, minimizing complications and improving surgical results.
